Adding a contact form in blogger is not that a big deal just a few steps and attention to details is required. here I have gathered one method I used for my Blogger website.
- First Create a New Page called Contact Us from blogger dashboard.
- Then select HTML Composing editor option under page title from left.
- delete all contents from the opened page.
- Past the code you will receive by email after submitting in the bellow form.
- Save and publish the page.
- you can access contact page at https://www.yourdomainname.com/p/contact-us.html
If you want your form inside your post just paste the html code in your post content
Note: when you add the form don't forget to replace the blogID value to be your blogID otherwise the data will not be submitted to your account, it will be sent to the account whose blogID is located in the form's code.

Lastly, if you are not satisfied with the form style you can change the CSS styling options if you are familiar with HTML and CSS language or completely redesign but keep the core functionality
